Metro Health Neighborhood Outpatient Centers Recognized as Patient-Centered Medical Home by Blue Cross Blue Shield of Michigan

Blue Cross Blue Shield of Michigan, or BCBSM, has recognized several Metro Health practices and neighborhood outpatient centers as patient-centered medical homes, or PCMH.

The patient-centered medical home is a model for care that promotes partnerships between individual patients and their personal physician by replacing episodic care based on illnesses with a more comprehensive approach to wellness and treatment. In this model, each patient’s care is managed by a physician-led care team, which provides for all health-care needs and coordinates treatment across the health care system.

About Metro Health Physician Hospital Organization

Metro Health Physician Hospital Organization, or PHO, is a group of primary and specialty care physicians along with Metro Health Hospital that partners with insurers, employers and patients to improve the health and well-being of our communities. Metro Health PHO began in 1995 as a partnership between Metro Health Hospital and Kent Health Care Physician Organization, or PO, and was the first organized PHO in West Michigan. The PHO brings providers together throughout the healthcare system and across wide geographic areas in an effort to better coordinate patient care. The physicians of Metro Health PHO believe that the patient should be the center of all health care. Through collaboration with Metro Health Hospital and the PO-employed and independently affiliated physicians, Metro Health PHO provides an environment that encourages the sharing of best practices, problem solving and process improvement.
