Case managers and social workers can help you have a safe discharge plan. They can also assist you and your family in dealing with social, emotional and financial issues that result from illness and hospitalization. If you have questions regarding your discharge plan, ask your nurse to contact your case manager or social worker.
Your health care team will assess and plan for continuing care needs whether you return home, transfer to an extended care facility or to another living situation. You will be given instructions about your post-hospital care. If you have questions about your diet, medications, activities or other matters, be sure to ask at the time of your discharge. If you think of questions following your discharge, contact your physician.
If insurance information or financial arrangements are incomplete, it may be necessary for you or a representative to stop at the Financial Counselor office before leaving the hospital.
Leaving the Hospital
When you leave, a staff member or volunteer will escort you to the lobby and help you into your vehicle.
Patients are randomly called by Press Ganey to answer questions about their hospital stay. This helps us monitor our patient satisfaction. We hope you will take the time to share your experience. Your feedback can help us improve our quality of care and services.
